1. How long will the survey take?
5-10mins. If you're very detailed perhaps a little longer. You will need to fill out the form in one sitting.
2. What if I don't want to share my last name?
If you're concerned about privacy issues but still want to contribute, then consider using your maiden name, middle name or your partner's maiden name to help protect your identity.
The case study is less credible when we only have a first name, which is why we strongly encourage you to include a last name of some kind.
3. How will the book be published?
The book will most likely be published as an ebook first. Depending on the feedback and the response from readers will determine if we look at other options further on.
4. When will the book be published?
We need to collect a reasonable number of case studies that provide a broad range of experience. We will also need to review, edit, format and and collate this information into an single document which will take some time. Depending on the response the book will hopefully be released sometime in Qtr 3, 2015.

Once you have submitted your application we will review it and get back to you if we have any questions.
As we progress further the final list of case studies to be included in the publication will be confirmed to all participants. Everyone who submits an application will be recognised and receive a copy of the publication.
